What’s the Idea?
We’re working to create collaborations and a shared system vision to develop a simple, person-centred, and trauma-aware wellbeing pathway for people in the South Tees area (and maybe across all of Tees if that is the way that the development naturally takes).
Here’s the vision for what we may suggest as some essential components of a wellbeing pathway:
A tool (s) you can use yourself to check and understand your individual wellbeing needs, with enhanced support if needed, as per current support pathways.
An easy-to-use, central online hub (A Wellbeing marketplace) with lots of options for improving wellbeing. We think this should work seamlessly from a self led wellbeing assessment and ‘cue up’ personalised support for people.
Considered enhanced ‘key worker’ type support at an early stage of the pathway for those who may struggle to self lead their own pathwway from assessment onwards.
The pathway and supporting resources are a universal offer, for the public, and referrers in the wellbeing system for much more reliable navigation.
This wellbeing pathway system should be simple, accessible, and helpful for everyone—whether they’re looking for support or helping others find it.
How Can You Help?

Imagine if…
…these features existed in a wellbeing pathway, wouldn’t that just be so much better?
Designed to offer a universal and holistic model, to assess, or self assess, in a trauma informed way, and provide personalised options for wellbeing support, in one place?
A pathway offers enhanced support roles, like social prescribers, mental health coaches or care navigators to guide those who need navigation support?
It gives system data to funders/commissioners of services about where need, movement and demand were happening , and then able to adjust resources to where they are needed?
Aa universal offer that anyone in South Tees could access, if able to, and complete a self-assessment to understand their own needs and aspirations for their futures? One example is offered by Clear Signal? (See link for demo on their site).
People actually access this pathway at an early stage and it supports demand reductions on primary and secondary healthcare services for something they are better set to access from a ‘wellbeing marketplace’?
Services that support housing, education, financial support, homelessness, health, social care, care homes, employment supporters could make use of this resource to offer a person centred offer for their students, customers, tenants, patients etc.
A clear pathway gives providers clear value of its functions and the benefits it brings to their own outcomes; giving incentive to take part and join a centralised model; a sustained model?
It can significantly reduce the burden on peoples time going round and round the system, and reduced pressures on ‘wrong doors’, as well as saving time for people working to support others?
Maybe, it starts small and grows with scalable approaches, and becomes a leading example to be championed nationally, to give understanding of peoples lives a priority seat, and reflect back to the public, the importance of self care, made much easier. with enhanced support for those who need it, and are more vulnerable to missing out.
We think the end result in a few years’ time, would be a ground-breaking integrated movement that promotes the best collaborations, with better security for services of many types based on clear need, a public health view of population needs and support and an answer to the question of complexity in person centred working.
